Witness Expenses Claim Form

Witnesses who have attended a Fitness to Practise inquiry at the Nursing and Midwifery Board of Ireland (NMBI) may be reimbursed for expenses such as accommodation, transport and meals by using the NMBI expenses claim form.

When you have completed the form, return it with any receipts to NMBI for processing and payment.
